Hospital-acquired pneumonia (HAP) is a leading cause of morbidity and mortality, commonly caused by Pseudomonas aeruginosa. Meropenem is a commonly used therapeutic agent, although emergent resistance occurs during treatment. We used a rabbit HAP infection model to assess the bacterial kill and resistance pharmacodynamics of meropenem. Meropenem 5 mg/kg administered subcutaneously (s.c.) q8h (±amikacin 3.33-5 mg/kg q8h administered intravenously[i.v.]) or meropenem 30 mg/kg s.c. q8h regimens were assessed in a rabbit lung infection model infected with P. aeruginosa, with bacterial quantification and phenotypic/genotypic characterization of emergent resistant isolates. The pharmacokinetic/pharmacodynamic output was fitted to a mathematical model, and human-like regimens were simulated to predict outcomes in a clinical context. Increasing meropenem monotherapy demonstrated a dose-response effect to bacterial kill and an inverted U relationship with emergent resistance. The addition of amikacin to meropenem suppressed the emergence of resistance. A network of porin loss, efflux upregulation, and increased expression of AmpC was identified as the mechanism of this emergent resistance. A bridging simulation using human pharmacokinetics identified meropenem 2 g i.v. q8h as the licensed clinical regimen most likely to suppress resistance. We demonstrate an innovative experimental platform to phenotypically and genotypically characterize bacterial emergent resistance pharmacodynamics in HAP. For meropenem, we have demonstrated the risk of resistance emergence during therapy and identified two mitigating strategies: (i) regimen intensification and (ii) use of combination therapy. This platform will allow pre-clinical assessment of emergent resistance risk during treatment of HAP for other antimicrobials, to allow construction of clinical regimens that mitigate this risk.IMPORTANCEThe emergence of antimicrobial resistance (AMR) during antimicrobial treatment for hospital-acquired pneumonia (HAP) is a well-documented problem (particularly in pneumonia caused by Pseudomonas aeruginosa) that contributes to the wider global antimicrobial resistance crisis. During drug development, regimens are typically determined by their sufficiency to achieve bactericidal effect. Prevention of the emergence of resistance pharmacodynamics is usually not characterized or used to determine the regimen. The innovative experimental platform described here allows characterization of the emergence of AMR during the treatment of HAP and the development of strategies to mitigate this. We have demonstrated this specifically for meropenem-a broad-spectrum antibiotic commonly used to treat HAP. We have characterized the antimicrobial resistance pharmacodynamics of meropenem when used to treat HAP, caused by initially meropenem-susceptible P. aeruginosa, phenotypically and genotypically. We have also shown that intensifying the regimen and using combination therapy are both strategies that can both treat HAP and suppress the emergence of resistance.

Shape memory alloys (SMAs) have become the most common choice for the development of mini- and micro-type soft bio-inspired robots due to their high power-to-weight ratio, ability to be installed and operated in limited space, silent and vibration-free operation, biocompatibility, and corrosion resistance properties. Moreover, SMA spring-type actuators are used for developing different continuum robots, exhibiting high degrees of freedom and flexibility. Spring- or any elastic-material-based antagonistic or biasing force is mostly preferred among all other biasing techniques to generate periodic oscillation of SMA actuator-based robotic body parts. In this model-based study, SMA-based spring-type actuators were used to develop a carangiform-type robotic fishtail. Fin size optimization for the maximization of forward thrust was performed for the developed system by varying different parameters, such as caudal fin size, current through actuators, pulse-width modulation signal (PWM), and operating depth. A caudal fin with a mixed fin pattern between the Lunate and Fork "Lunafork" and a fin area of approximately 5000 mm2 was found to be the most effective for the developed system. The maximum forward thrust developed by this fin was recorded as 40 gmf at an operation depth of 12.5 cm in a body of still water.

Skin cancer, encompassing various forms such as melanoma, basal cell carcinoma, and others, remains a significant global health concern, often proving fatal if not diagnosed and treated in its early stages. The challenge of accurately diagnosing skin cancer, particularly melanoma, persists even for experienced dermatologists due to the intricate and unpredictable nature of its symptoms. To address the need for more accurate and efficient skin cancer detection, a novel Golden Hawk Optimization-based Distributed Capsule Neural Network (GHO-DCaNN) is proposed. This novel technique leverages advanced computational methods to improve the reliability and precision of skin cancer diagnosis. An optimized clustering-based segmentation approach is introduced, integrating the innovative Sewer Shad Fly Optimization (SSFO), which combines elements of both mayfly and moth flame optimization. This integration enhances the accuracy of lesion boundary delineation and feature extraction. The core of the innovation lies in the optimized distributed capsule neural network, which is trained using the Hybrid GHO. This optimizer, inspired by the behaviors of the golden eagle and fire hawk, ensures the effectiveness of epidermis lesion detection, pushing the boundaries of skin cancer diagnosis methods. The achievements based on the metrics, like specificity, sensitivity, and accuracy show 97.53%, 99.05%, and 98.83% for 90% of training and 97.83%, 99.50%, and 99.06% for k-fold of 10, respectively.

Bacterial persisters represent non-inheritable drug tolerant population that are linked to recalcitrance of infections in healthcare settings. The rise of antibiotic resistance and depletion of new antibiotics in drug discovery pipeline has made the task of persister eradication more daunting. In the present study, we report that treatment of Acinetobacter baumannii with the last resort antibiotic polymyxin B displays continuous variation in tolerance among different clinical isolates. Mechanistically, Polymyxin B persisters exhibit disruption of proton motive force led delocalisation of cell division protein to attain a growth arrested phenotype. Tolerance studies on mutant strains revealed that superoxide dismutase (sodB) activity is a major contributor in tolerance of A. baumannii to polymyxin B. Using a dual fluorescence based persister detection system, screening of various antibiotics was performed to eradicate polymyxin B induced persisters of A. baumannii Rifampicin exhibited eradication of polymyxin B tolerant population by log reduction of 6 in magnitude against different clinical isolates of A. baumannii We establish that enhanced generation of ROS by rifampicin leads to clearance of these polymyxin B persisters. It was further demonstrated, as a proof of concept, that rifampicin potentiates the killing of polymyxin B persisters in murine wound infection model. We found that the effects were linked to significant down regulation of sodB by rifampicin, which contributes to higher generation of ROS in polymyxin B tolerant cells. In view of these results, we propose that the combination of polymyxin B and rifampicin is an effective antipersister strategy in clearing polymyxin B induced A. baumannii persisters.

Introduction: Pyogenic granuloma is a commonly occurring inflammatory hyperplasia involving skin and mucous membranes. Various modalities of treatment have been used to treat pyogenic granuloma. However, there is an increased risk of intraoperative bleeding and recurrence of pyogenic granuloma following surgical treatment of pyogenic granuloma. Therefore, sclerotherapy has evolved as an effective alternative treatment modality in excellent safety and efficacy. Aims and Objectives: The aim of this study was to assess the safety and efficacy of 3% polidocanol in liquid form in pyogenic granuloma as a sclerosant. Settings and Design: This was a retrospective study of cases treated between March 2019 and February 2020 at two different private institutes. Materials and Methods: The study included 30 patients with 30 pyogenic granulomas treated with 3% polidocanol liquid. Individuals with comorbid conditions such as diabetes, hypertension, thyroid disorders, and those who were on medications were excluded from the study. Two units of 3% polidocanol solution were injected with an insulin syringe at the base of the lesion. Changes in lesions and adverse events were recorded and injections were repeated after a gap of 2 weeks if needed. Result: In 28 patients, there was complete resolution of the lesion within 4 weeks and 2 patients received a second injection of polidocanol. All the patients tolerated the procedure and the lesions resolved without any significant sequelae. Discussion: The advantages of 3% polidocanol sclerotherapy are that it is a safe, easy, effective, and minimally invasive procedure with little discomfort to the patient and very minimal complications as compared with other modalities. Conclusion: Polidocanol 3% solution is an effective sclerosant for the treatment of pyogenic granuloma.The aim of this study was to assess the safety and efficacy of 3% polidocanol in the liquid form in PG as a sclerosant.

OBJECTIVES: To evaluate the in vitro and in vivo efficacy of the FDA-approved drug disulfiram in combination with meropenem against MBL-expressing carbapenem-resistant Acinetobacter baumannii. METHODS: Chequerboard and antibiotic resistance reversal analysis were performed using 25 clinical isolates producing different MBLs. Three representative strains harbouring NDM, IMP or non-MBL genes were subjected to a time-kill assay to further evaluate this synergistic interaction. Dose-dependent inhibition by disulfiram was assessed to determine IC50 for NDM-1, IMP-7, VIM-2 and KPC-2. Further, to test the efficacy of meropenem monotherapy and meropenem in combination with disulfiram against NDM- and IMP-harbouring A. baumannii, an experimental model of systemic infection and pneumonia was developed using BALB/c female mice. RESULTS: Chequerboard and antibiotic reversal assay displayed a synergistic interaction against MBL-expressing A. baumannii strains with 4- to 32-fold reduction in MICs of meropenem. In time-kill analysis, meropenem and disulfiram exhibited synergy against NDM- and IMP-producing carbapenem-resistant A. baumannii (CRAb) isolates. In vitro dose-dependent inhibition analysis showed that disulfiram inhibits NDM-1 and IMP-7 with IC50 values of 1.5 ± 0.6 and 16.25 ± 1.6 µM, respectively, with slight or no inhibition of VIM-2 (<20%) and KPC-2. The combination performed better in the clearance of bacterial load from the liver and spleen of mice infected with IMP-expressing CRAb. In the pneumonia model, the combination significantly decreased the bacterial burden of NDM producers compared with monotherapy. CONCLUSIONS: These results strongly suggest that the combination of disulfiram and meropenem represents an effective treatment option for NDM- and IMP-associated CRAb infections.

Abstract Risperidone is an atypical antipsychotic drug widely prescribed all over the world due to its clinical advantages. The currently available long acting marketed depot formulation of risperidone is a microsphere based preparation using poly-[lactide-co-glycolide] (PLGA) as drug release barrier. It is however, a cold chain product due to thermal instability of PLGA at room temperature. After beginning the depot injection therapy it is administered every two weeks but associated with another drawback of about 3 weeks lag time due to which its tablets are also administered for three weeks so as to attain and maintain therapeutic drug concentration in the body. The present work attempts to develop a long acting depot delivery system of risperidone for once a month administration based on the combination of sucrose acetate isobutyrate and polycaprolactone dissolved in benzyl benzoate to provide an effective drug release barrier for one month without any lag time and which can be stored at room temperature precluding the requirement of cold supply chain. The developed depot formulation showed a sustained in vitro drug release profile with 88.95% cumulative drug release in 30 days with little burst release. The in vivo pharmacokinetic studies of the developed formulation conducted on rats showed attainment of mean peak plasma drug concentration of 459.7 ng/mL in 3 days with a mean residence time of 31.2 days, terminal half-life of 20.6 days, terminal elimination rate constant of 0.0336 per day, and a good in vitro- in vivo correlation.

The enormous use of cutting fluid in machining leads to an increase in machining costs, along with different health hazards. Cutting fluid can be used efficiently using the MQL (minimum quantity lubrication) method, which aids in improving the machining performance. This paper contains multiple responses, namely, force, surface roughness, and temperature, so there arises a need for a multicriteria optimization technique. Therefore, in this paper, multiobjective optimization based on ratio analysis (MOORA), VIseKriterijumska Optimizacija I Kompromisno Resenje (VIKOR), and technique for order of preference by similarity to ideal solution (TOPSIS) are used to solve different multiobjective problems, and response surface methodology is also used for optimization and to validate the results obtained by multicriterion decision-making technique (MCDM) techniques. The design of the experiment is based on the Box-Behnken technique, which used four input parameters: feed rate, depth of cut, cutting speed, and nanofluid concentration, respectively. The experiments were performed on AISI 304 steel in turning with minimum quantity lubrication (MQL) and found that the use of hybrid nanofluid (Alumina-Graphene) reduces response parameters by approximately 13% in forces, 31% in surface roughness, and 14% in temperature, as compared to Alumina nanofluid. The response parameters are analyzed using analysis of variance (ANOVA), where the depth of cut and feed rate showed a major impact on response parameters. After using all three MCDM techniques, it was found that, at fixed weight factor with each MCDM technique, a similar process parameter was achieved (velocity of 90 m/min, feed of 0.08 mm/min, depth of cut of 0.6 mm, and nanoparticle concentration of 1.5%, respectively) for optimum response. The above stated multicriterion techniques employed in this work aid decision makers in selecting optimum parameters depending upon the desired targets. Thus, this work is a novel approach to studying the effectiveness of hybrid nanofluids in the machining of AISI 304 steel using MCDM techniques.

Small RNAs in bacteria are noncoding RNAs that act as posttranscriptional regulators of gene expression. Over time, they have gained importance as fine-tuners of expression of genes involved in critical biological processes like metabolism, fitness, virulence, and antibiotic resistance. The availability of various high-throughput strategies enable the detection of these molecules but are technically challenging and time-intensive. Thus, to fulfil the need of a simple computational algorithm pipeline to predict these sRNAs in bacterial species, we detail a user-friendly ensemble method with specific application in Acinetobacter spp. The developed algorithms primarily look for intergenic regions in the genome of related Acinetobacter spp., thermodynamic stability, and conservation of RNA secondary structures to generate a model input for the sRNAPredict3 tool which utilizes all this information to generate a list of putative sRNA. We confirmed the accuracy of the method by comparing its output with the RNA-seq data and found the method to be faster and more accurate for Acinetobacter baumannii ATCC 17978. Thus, this method improves the identification of sRNA in Acinetobacter and other bacterial species.

Acinetobacter baumannii is a Gram-negative nosocomial pathogen that causes soft tissue infections in patients who spend a long time in intensive care units. This recalcitrant bacterium is very well known for developing rapid drug resistance, which is a combined outcome of its natural competence and mobile genetic elements. Successful efforts to treat these infections would be aided by additional information on the physiology of A. baumannii Toward that end, we recently reported on a small RNA (sRNA), AbsR25, in this bacterium that regulates the genes of several efflux pumps. Because sRNAs often require the RNA chaperone Hfq for assistance in binding to their cognate mRNA targets, we identified and characterized this protein in A. baumannii The homolog in A. baumannii is a large protein with an extended C terminus unlike Hfqs in other Gram-negative pathogens. The extension has a compositional bias toward glycine and, to a lower extent, phenylalanine and glutamine, suggestive of an intrinsically disordered region. We studied the importance of this glycine-rich tail using truncated versions of Hfq in biophysical assays and complementation of an hfq deletion mutant, finding that the tail was necessary for high-affinity RNA binding. Further tests implicate Hfq in important cellular processes of A. baumannii like metabolism, drug resistance, stress tolerance, and virulence. Our findings underline the importance of the glycine-rich C terminus in RNA binding, ribo-regulation, and auto-regulation of Hfq, demonstrating this hitherto overlooked protein motif to be an indispensable part of the A. baumannii Hfq.

In India, freshwater aquatic resources are suffering from increasing human population, urbanization and shortage of all kind of natural resources like water. To mitigate this, all the major rivers have been planned for a river-interlinking through an interlinking canal system under a huge scheme; yet, the baseline information on ecological conditions of those tropical rivers and their fish communities is lacking at present. In view of that, the present study was undertaken to assess the ecological condition by comparing the trophic metrics of the fish community, conservation status and water chemistry of the two tropical rivers of the Ganga basin, from October 2007 to November 2009. The analysis of trophic niches of the available fish species indicated dominancy of carnivorous (19 species) in river Ken and omnivorous (23 species) in Betwa. The trophic level score of carnivorous species was recorded similar (33.33%) in both rivers, whereas omnivorous species were mostly found in Betwa (36.51%) than Ken (28.07%). Relatively undisturbed sites of Betwa (B1, B2 and B3) and Ken (K2, K3 and K5) were characterized by diverse fish fauna and high richness of threatened species. The higher mean trophic level scores were recorded at B4 of Betwa and K4 of Ken. The Bray-Curtis index for trophic level identified the carnivorous species (> 0.32) as an indicator species for pollution. Anthropogenic exposure, reflected in water quality as well as in fish community structure, was found higher especially in the lower stretches of both rivers. Our results suggest the importance of trophic metrics on fish community, for ecological conditions evaluation, which enables predictions on the effect of future morphodynamic changes (in the post-interlinking phases), and provide a framework and reference condition to support restoration efforts of relatively altered fish habitats in tropical rivers of India.

En la India, los recursos acuáticos de agua dulce están sufriendo debido a la creciente población humana, la urbanización y la escasez de todo tipo de recursos naturales como el agua. Para mitigar esto, se ha planificado bajo un amplio esquema que todos los grandes ríos estén interconectados a través de un sistema de canales, sin embargo, la información básica sobre las condiciones ecológicas de los ríos tropicales y sus comunidades de peces es escasa en la actualidad. En vista de ello, el presente estudio se realizó para evaluar el estado ecológico, mediante la comparación de los parámetros tróficos de la comunidad de peces, estado de conservación y el agua de los dos ríos tropicales de la cuenca del Ganges, de octubre 2007 a noviembre 2009. El análisis de los nichos tróficos de las especies de peces disponibles indican dominancia de carnívoros (19 especies) en el río Ken y omnívoros (23 especies) en Betwa. El nivel trófico de las especies carnívoras fue similar en ambos ríos (33.33%), mientras que las especies omnívoras fueron mayores en Betwa (36.51%) que en Ken (28.07%). Sitios relativamente inalterados de Betwa (B1, B2 y B3) y Ken (K2, K3 y K5) se caracterizaron por la alta diversidad y riqueza de especies de peces amenazadas. Los puntajes medios más altos del nivel trófico se registraron en B4 de Betwa y K4 de Ken. El índice de Bray-Curtis para el nivel trófico identificó las especies carnívoras (>0.32) como indicadoras de contaminación. Exposición antropogénica, reflejada en la calidad del agua, así como en la estructura de la comunidad de peces, fue más alta, especialmente en los tramos inferiores de ambos ríos. Nuestros resultados sugieren la importancia de métricas tróficas en la comunidad de peces, para la evaluación de las condiciones ecológicas, lo que permite hacer predicciones sobre el efecto de futuros cambios morfodinámicos (en las fases de post-interconexión), y proporcionar un marco y condición de referencia para apoyar los esfuerzos de restauración de hábitat de peces relativamente alterados en los ríos tropicales de la India.

In India, freshwater environments are experiencing serious threats to biodiversity, and there is an urgent priority for the search of alternative techniques to promote fish biodiversity conservation and management. With this aim, the present study was undertaken to assess the fish biodiversity within and outside a river protected area, and to evaluate whether the protected river area provides some benefits to riverine fish biodiversity. To assess this, the pattern of freshwater fish diversity was studied in river Gerua, along with some physicochemical conditions, from April 2000 to March 2004. For this, a comparison was made between a 15km stretch of a protected area (Katerniaghat Wildlife Sanctuary), and an unprotected one 85km downstream. In each site some physicochemical conditions were obtained, and fish were caught by normal gears and the diversity per site described. Our results showed that water temperature resulted warmest during the pre-monsoon season (25 degreeC) and low during the winter (14-15 degreeC); turbidity considerably varied by season. In the protected area, a total of 87 species belonging to eight orders, 22 families and 52 genera were collected; while a maximum of 59 species belonging to six orders, 20 families and 42 genera were recorded from the unprotected areas. Cyprinids were found to be the most dominant genera and Salmostoma bacaila was the most numerous species in the sanctuary area. Other numerous species were Eutropiichthys vacha, Notopterus notopterus, Clupisoma garua and Bagarius bagarius. The results indicated more species, greater abundances, larger individuals, and higher number of endangered fishes within the sanctuary area when compared to the unprotected area. Analysis on the mean abundance of endangered and vulnerable species for the evaluated areas in the sanctuary versus unprotected ones indicated significant differences in fish abundance (p<0.05). These results showed that this riverine protected area could be important for conservation and management of fish diversity in the region, especially for resident and threatened species.

En India los ambientes de agua dulce están experimentando una grave amenaza para la biodiversidad; ante esto es urgente investigar técnicas alternativas que promuevan la biodiversidad y manejo de los peces. El presente estudio se realizó con el objetivo de evaluar la biodiversidad de los peces dentro y fuera del área protegida del río Gerua, India, para evaluar si el área protegida provee beneficios a la biodiversidad de los peces fluviales. Para evaluar el patrón de diversidad de los peces de agua dulce y las condiciones físico-químicas se estudió el río entre abril 2000 y marzo 2004. Se realizó una comparación en un tramo de 15km de un área protegida (Santuario de Vida Silvestre Katerniaghat) y un tramo sin protección de 85km aguas abajo. En cada sitio se obtuvieron algunas variables fisicoquímicas, los peces fueron capturados con varias artes de pesca y se describió la diversidad por sitio. Nuestros resultados mostraron que la temperatura del agua es mayor como resultado de la temporada de pre-monzón (25ºC) y menor durante el invierno (14-15ºC), la turbidez varía considerablemente según la temporada. En el área protegida, se recolectaron un total de 87 especies pertenecientes a ocho órdenes, 22 familias y 52 géneros, mientras que un máximo de 59 especies pertenecientes a seis órdenes, 20 familias y 42 géneros se registraron en las áreas no protegidas. El género más abundante pertenece a los Ciprínidos y Salmostoma bacaila fue la especie más abundante en el área del santuario. Otras especies numerosas fueron: Eutropiichthys vacha, Notopterus notopterus, Clupisoma garúa y Bagarius bagarius. Los resultados indicaron más especies, mayores abundancias, individuos más grandes y mayor número de peces con categoría de peligro de extinción dentro del área del santuario cuando se compara con la zona no protegida. El análisis de la abundancia promedio de peces en peligro de extinción y vulnerables de las áreas evaluadas en el santuario frente a las desprotegidos indicó diferencias significativas (p<0.05). Los resultados mostraron que esta área protegida ribereña podría ser importante para la conservación y manejo de la diversidad de los peces en la región, especialmente para los residentes y las especies amenazadas.
